In order to improve as a volunteer run youth sport organization, we would like the feedback of the parents and players to assess the effectiveness and abilities of our coaching staff over the past season. It is important that you are honest and fair in your assessment.
Please complete this form at any time during the season. It is important that they be received prior to Coach Selection for the following year. Coach Selection is completed February 15 - 29. The earlier a concern is brought to our attention, the earlier we can address it. Please don't leave any serious issue to the end of the season. Contact any member of the executive if you have a serious issue that needs addressing.

Lack of evaluations may lead to a good coach not being selected, so positive evaluations are very important. Similarly, coaching concerns need to be brought to our attention.
All personal information is kept confidential, with the exception of the President of SurDel. Telephone numbers would be helpful if the president would like to followup on any comments or concerns. Evaluations will then be summarized and all personal or identifying information removed before sharing with the Coach and Coach Selection Committee.
Evaluations without personal information will be reviewed by the President but will not be provided to the Coach Selection Committee.
